 What is the Service?

The Chronic Wound Service provides treatment to patients in the community with chronic or complex wounds. Services are offered centre based and home based including Residential Aged Care Facilities.

What can I expect from the service?

Clients undergo a comprehensive assessment including diagnosis, treatment, advice and education. This service is staffed by a Geriatrician, Wound Nurse Consultant, Dietetics and Podiatry.

The Chronic Wound Service is held at 3 Northern Health sites:

Broadmeadows Hospital:
Clinic operates on Tuesday afternoon
Home visits completed on Tuesday morning and Thursday
Bundoora:
Clinic operates on Thursday
Home Visits completed on Wednesday and Friday
Craigieburn:
Clinic operates on Monday
Home visits on Thursday

What do I bring/need for my appointment?
Please bring a list of current medications, relevant scans or results.

How much does it cost?

  • No cost for attending the Chronic Wound Service.
  • If client is referred onto additional Community Therapy Services, there may be a fee incurred of $5-10 per visit.
  • If client is ineligible for Medicare the referral for the service will need to be reviewed by management

Inclusion criteria:
A persistent area of broken skin, or a wound with delayed healing that has been present for more than 28 days.

Exclusion criteria:
Recent surgical wound less than 1 month duration.

How to Refer:

Referral from any source will be accepted. Referral must include:

  • Reason for referral
  • Current and past medical history
  • Information relation to the patient’s condition
