Organise an assessment with your local Aged Care Assessment Service (ACAS) via www.myagedcare.gov.au or phone: 1800 200 422

Receive approval for a Home Care Package and await Home Care Package “assignment” from the My Aged Care National Queue.

Receive confirmation letter from My Aged Care assigning you Home Care Package and referral code.

Select Northern Health Home Care Packages as your Home Care Package Service Provider. Simply contact us!
