About us
Residential In Reach (RIR) offers specialty health care assessment and a rapid response service for people with complex medical conditions who are living in Residential Aged Care Facilities.
The team comprises Geriatricians and Palliative Care specialists, Aged Care and Medical Registrars, a Nurse Practitioner and Aged Care Clinical Nurse Consultants.
The team works closely with residents, families, Residential Care Facility staff and General Practitioners. The primary aim of the service is to optimise the resident’s health and wellness and prevent future avoidable hospital admissions.
Residents are assisted to achieve their goals considering future health changes and decisions required with Advance Care Directives.
Services we provide
Assessments and interventions may include:
- comprehensive health assessment
- review of medications
- organising of tests (e.g. chest X-ray, urine tests, blood tests)
- provision of medications and/or fluids
- family support meetings
- education about medical conditions
- supporting facility staff to identify early clinical deterioration and to action planned care in facilities
There is no cost for the service.
Facilities we service
The Residential In Reach (RIR) team currently provides telehealth and visiting services to the following facilities.
The team is also able to provide telehealth services to facilities in rural Victoria.
How to refer
Following receipt of referral, referrals are triaged and processed within 24 – 48 hours. Processing involves checking eligibility, confirming referral information and triaging the priority of the referral. To refer a patient, please complete the online referral form.
